Community Services Block Grant Program
Program Name: Self-Sufficiency
Poverty Cause Name: Inadequate Income
Objective: To provide comprehensive case management service and activities to 50 families for a two year period by continuing at least 30 carry-over families.
Long Range Goals: To reduce the number of families in poverty by 50 families within a two year period of time.
Program Qualification: Persons/families seeking admission to the self-sufficiency program must be qualified by current HHS poverty income guidelines for family units. Qualification for enrollment in the program will be determined by the income verification process effective the date of application, counting backward a full year.
Project Activities: Services provided to families enrolled in the program is based on a need and a plan of action agreed to and executed between the family and our agency.
Supportive Services: Financial assistance is available to families on a demonstrated need basis. Each request stands alone and is considered on its on merit.
